Primary Care Networks (PCNs) have been established since July 1st, 2019, forming a collective of GP practices dedicated to close collaboration with primary and community care professionals and healthcare organisations. Together, we aim to offer seamlessly integrated healthcare services to our local communities and patients.
Our PCN is known as Hills, Brook and Dales PCN (HBD PCN) and comprises of seven GP Practices and serves over 60,000 patients.
These are: Brixton Hill Group Practice, Herne Hill Group Practice, Northwood Group Practice, Knights Hill Surgery, The Deerbrook Surgery, Lambeth Healthcare, and Prentiss Road Surgery.
The Clinical Director is Dr Sadru Kheraj.
