What is self-referral?

Self-referral allows you to directly access specialised care without the need for a referral from your GP. Whether you’re seeking physiotherapy, mental health support, or maternity services, you can take the initiative to schedule an appointment yourself. By offering self-referral pathways, we aim to streamline the process and ensure that you receive the care you need in a timely manner.

SafeLives

SafeLives is a UK-wide charity dedicated to ending domestic abuse, for everyone and for good.  

If you are experiencing domestic abuse or are supporting someone who is in that situation, immediate help is available.


Refuge & National Domestic Violence Helpline

Refuge is an independent, specialist charity that supports women fleeing domestic abuse and other forms of gender-based violence. They have been doing this work since 1971. Their specialist services now support around 6,500 women and children on any given day.

The Community Adult Audiology 

This service is for people who are experiencing hearing difficulty as a result of normal ageing. The Community Adult Audiology self-referral service is available to patients:

• Who are Registered with a SEL GP Practice

• Who are Over 18 Years of Age 

• Who are suitable for the Community Adult Audiology service

Community Adult Audiology is NOT suitable for all patients. To help patients to determine if they are suitable for self-referral, SEL Community Adult Audiology team have created a patient self-referral check list.

Bereavement

Child Bereavement UK

Child Bereavement UK help children and young people (up to age 25), parents, and families, to rebuild their lives when a child grieves or when a child dies. They offer free, confidential bereavement support by telephone, video or instant messenger, wherever you live in the UK as well as face-to-face support from a number of locations.


SANDS (Stillbirth & Neonatal Death Charity)

SANDS, the stillbirth and neonatal death charity, is a well-established and widely respected UK charity that:

  • Supports anyone affected by the death of a baby
  • Works in partnership with health professionals to ensure that bereaved parents and families receive the best possible care
  • Promotes and funds research and changes in care that could help to reduce the loss of babies’ lives.

SANDS has a network of over 100 Groups around the UK, and 12 in Scotland. Groups offer support to anyone affected by the death of a baby and they work closely with their local hospitals to ensure the best possible care is provided and promote awareness and the work of Sands nationally.


TCF (The Compassionate Friends)

The Compassionate Friends (TCF) is a charitable organisation of bereaved parents, siblings and grandparents dedicated to the support and care of other similarly bereaved family members who have suffered the death of a child or children from a month old and from any cause.


Brake

Brake is a road safety charity working with communities and organisations across the UK to stop the tragedy of road deaths and injuries, make streets and communities safer for everyone, and support people bereaved and seriously injured on roads. They have been working since 1995 to make a difference across the UK.


Scotty’s Little Soldiers

Scotty’s Little Soldiers is a charity dedicated to supporting children and young people who have lost a parent serving in the British Armed Forces.

Hope Again

Hope Again is the youth website of Cruse Bereavement Care. It is a safe place where you can learn from other young people, how to cope with grief, and feel less alone. The Cleft Lip and Palate Association (CLAPA) is the national charity supporting people born with a cleft and their families in the UK.

We bring together people affected by cleft to help them connect with others who share their experiences, welcoming them into a supportive community for life.

Living in a cold home can negatively impact your health and overall well-being. The South London Stay Warm and Well Network offers practical assistance to vulnerable households facing the threat of cold temperatures. They can help you reduce heating costs and ensure warmth during the winter months. You can self-refer to the South London Healthy Homes program for support.

Counselling and Mental Health

Lambeth Talking Therapies (IAPT)

Lambeth Talking Therapies is a part of the Improving Access to Psychological Therapies Service (IAPT). Their service is available to anyone 18 years and older registered with a GP in Lambeth.

They offer talking therapies for people experiencing mild to depression, anxiety, worry, panic attacks, traumatic memories and obsessive-compulsive disorder. They also offer help with other problems including anger, eating and relationship or sexual difficulties.

If their service isn’t right for you, they’ll suggest alternatives that are more likely to meet your needs and put you in contact with an appropriate service. 

SELF-REFERRAL

A self-referral offers you access to a specialist without the need to see your GP first. A self-referral may also reduce your wait to access an expert opinion, advice and treatment of your condition and puts you in control of your care.

Help With Using Digital Services

Lambeth Together

Lambeth Together is a partnership of the voluntary and community sector, the NHS, South East London Integrated Care System (ICS), Lambeth Council, and others, focused on improving health and wellbeing and reducing inequalities for people in Lambeth through an integrated health and care system.

Working together, they coordinate care across our borough to remove unhelpful divides between hospital and community-based services, physical and mental health, and health and social care.

As part of their support, they offer advice and guidance in Lambeth to learn how to start using the internet to manage your health appointments, access your medical records online and much more.

Home Modifications Due To Health

Home Adaptations – Lambeth Council

Home adaptations are changes you can make to your home. These make it safer and easier to move around and do everyday tasks.

Your local council offers a service that assesses your home and recommends changes to help. Having a home assessment is free.

 These changes can be small or big and include:

  • fitting a stairlift or a banister on the stair
  • adding a bath lift, walk-in shower or a rail you hold to pull yourself out of the bath (grab rail)
  • widening doorways
  • lowering kitchen worktops
  • putting in an outdoor ramp or step rail
  • security, such as outside lights and intercom systems

The assessment might also recommend household equipment and gadgets to make life easier.

Crisis are the national charity for people experiencing homelessness. They help people out of homelessness and campaign for the changes needed to solve it altogether.

Crisis provide practical support to help people access benefits, healthcare services, employment opportunities, and more. Their main aim is to relieve the huge pressure of homelessness, by helping people find safe and affordable homes as quickly as possible.

They prioritise supporting people who are facing the most complex barriers to ending their homelessness and often have nowhere else to turn.

Shelter exist to defend the right to a safe home. Because home is everything.

The National Childbirth Trust are a charity that supports people as they become parents. They support parents through pregnancy, birth and early parenthood.


ICON

ICON is all about helping people who care for babies to cope with baby crying. Whilst, baby crying is normal, it can be stressful and overwhelming for the parent or carer.  ICON aims to help you cope by offering guidance and advice.  Please visit their website for more information.

The Lullaby Trust

The Lullaby Trust raises awareness of sudden infant death syndrome (SIDS), provides expert advice on safer sleep for babies and offers emotional support for bereaved families.


Twins Trust / TAMBA (Twins and Multiple Births Association)

The Twins trust is a charity set up by parents of twins, triplets and higher multiples and interested professionals in 1978. It is the only UK wide organisation that directly helps parents and professionals to meet the unique challenges that multiple birth families face.

Every 10 seconds, Samaritans responds to a call for help. Their free helpline is open, day or night, 365 days a year for anyone who’s struggling to cope, who needs someone to listen without judgement or pressure.

Samaritans is not only for the moment of crisis, but they’re also taking action to prevent the crisis.

They give people ways to cope and the skills to be there for others. They also encourage, promote and celebrate those moments of connection between people that can save lives.

Samaritans offer listening and support to people and communities in times of need.


Combat Stress

Combat Stress is the UK’s leading veterans’ mental health charity, the work they do is life-changing and often lifesaving. Their services are provided across the UK, in-person and online. At a time that can be isolating and daunting, they’re here to help.

They provide clinical treatment and support for veterans from the British Armed Forces, with a focus on those with complex mental health issues. When some military personnel leave service, their experiences can’t easily be left behind. Without treatment, psychological trauma can eventually tear lives apart.

Combat Stress believe military trauma shouldn’t destroy the lives of veterans and their families. For over a century, they’ve helped former servicemen and women with mental health problems such as post-traumatic stress disorder (PTSD), anxiety and depression.


CALM (Campaign against living miserably)

The Campaign Against Living Miserably (CALM) is a registered charity based in England who are leading movement against living miserably and taking a stand against suicide.

They stand together with everyone who’s struggling with life, no matter who they are, where they’re from or what they’re going through. And they do it through their life-saving services, national campaigns, and by building communities.

Minor Eye Conditions Service (MECS) 

Minor Eye Conditions Service (MECS)

The Minor Eye Conditions Service allows patients across Lambeth, Southwark and Lewisham to access NHS treatment for a range of eye conditions. Instead of having to visit hospital for treatment, you can now be seen by a highly trained optometrist based in a local optician’s practice.

Aim of the Scheme?

To allow all patients to be able to access treatment quickly, near where you live and work. Appointments are often available within two days, and many of the practices offer appointments at weekends.

Non-urgent advice: Note:

If you are suffering from significant pain, a sudden loss of vision in one or both eyes, chemical burns or a penetrating injury then you should go directly to the Eye Casualty department at St Thomas’ Hospital or your nearest A&E.

Medical eye conditions that can be treated by MECS: 

  • Sudden or recent reduction in vision in one or both eyes 
  • Red eye(s) or eye lids 
  • Pain and / or discomfort in the eyes, around the eye area or temples 
  • Recent onset or sudden increase of flashes and / or floaters in one or both eyes (appears like a fly, black specks or a cobweb moving across your vision) 
  • Mild trauma, for example a scratch to the outer surface of the eye(s) or lid(s) 
  • Suspected foreign body in the eye 
  • Recent onset of double vision 
  • Significant recent discharge from or watering of the eye

Minor Illnesses & Minor Injuries

For common illnesses and minor health concerns consider self-care or visit your local pharmacy.

Self Care 

Self Care is the best choice to treat minor illnesses and injuries such as coughs and colds, diarrhoea, grazes etc. A range of common illnesses and injuries can be treated at home simply by combining a well-stocked medicine cabinet with plenty of rest.

Pharmacies 

It is government policy to reduce the amount of money the NHS spends on prescriptions for treating minor conditions. GPs do not generally give prescriptions to treat minor health conditions that usually get better on their own.

Instead, over the counter (OTC) medicines are available to buy in a pharmacy or supermarket.

NHS 111 is the NHS non-emergency number. It’s fast, easy and free. Call 111 and speak to a highly trained adviser, supported by healthcare professionals.

Calls are free from landlines and mobile phones.

You should use the NHS 111 service if you urgently need medical help or advice but it’s not a life-threatening situation.​

A&E

Accident and Emergency (A&E) departments and 999 should only be used in a critical or life-threatening situation. A&E departments provide immediate emergency care for people with very serious or life-threatening illness or injury.

It can be accessed on a Smartphone or on the Web and guides you day-by-day through a sequence of exercises and tips to help you get better from a range of new or recurrent musculoskeletal conditions:

What is a Self-Referral?

A self-referral offers you access to a specialist without the need to see your GP first. A self-referral may also reduce your wait to access an expert opinion, advice and treatment of your condition and puts you in control of your care. 

For ongoing foot health issue/conditions, you can complete a self referral form at Guy’s and St Thomas’ – The community podiatry (foot health) clinic. They offer a range of services to assess, treat and advise people on any foot condition and will help you to:

  • Prevent future foot problems through health promotion
  • Relieve pain
  • Maintain function and mobility
  • Prevent or minimise deformity
  • Reduce the risk of ulceration and amputation

Emergency Walk-In Foot Clinics

For urgent foot health conditions, there are emergency walk-in foot health clinics for patients who need emergency attention.

They treat:

  • foot ulceration, wounds and sores
  • pre-ulcerative areas on the feet (hard skin or corns which look bruised or are yellow, green, red or white)
  • bleeding or pus in the toenails or feet
  • hot, red, swollen or infected feet
  • minor burns
  • injury from foreign bodies such as glass, wood, a nail or needle
  • wound-redressing or nail surgery-redressing.

The emergency clinic may close earlier if the capacity for the clinic has been reached.

Pregnancy and Antenatal Care Self-Referral

Local pharmacy service

Participating pharmacies offer face-to-face or remote support as well as nicotine replacement therapy (NRT) or medication. You’ll be able to access free one-to-one support during the first six weeks of your journey to become smoke-free, along with up to 12 weeks’ worth of NRT supply and Champix.

Lambeth Stop Smoking service

You can refer yourself to the specialist Stop Smoking service if you are from one of the ‘at-risk’ groups below:

  • Diagnosed with one or more long-term conditions, including mental illness.
  • Pregnant women and their partners.
  • Parents with children aged five or under.
  • Smokers considered to be complicated cases, such as chronic smokers.

During your initial appointment, your Stop Smoking specialist will take you through the stages of stopping, including preparing to stop, actually stopping and staying stopped.

You’ll have a minimum of six weeks’ free one-to-one behavioural support in your journey to become smoke-free. Your specialist will help and support you in your choice and use of Nicotine Replacement Therapy (NRT) or Champix medication. You can access this service face to face or remotely.
